It is helpful to have a list of medications that you are presently taking with you when you see your doctor. Below are some links to resources that are available. Note that most of the forms below DO NOT allow you to save a copy on your computer. Once you have filled out the form you are done. On My computer the fda.gov appears to be an exception to this, although the site states that this is possible if you have Adobe Professional software on your computer.
