Admission Qualifications
-Individual must have a documented brain injury which is currently a barrier to employment and/or independent living.
-Individual must have means of financial support which will cover any cost incurred in the program.
-Individual must be eligible for services from a service provider committing to the long term support component of Crumley House services or have other means of financial support. (Only if the services are deemed necessary in order to be successful on the job and/or in an assisted living situation)
-Individual must be at least 18 years old.
-The individual and primary care provider, where appropriate, must be demonstrate a willingness to participate in The Crumley House program and view competitive employment as a service outcome.
-The individual and primary care provider, where appropriate, must be willing to meet with the Social Security Administration Representative, if benefits are being received, prior to job placement.
-The individual and primary care provider, where appropriate, must be willing to be committed to an on-going process of Personal Futures Planning.
-The individual must have had a neuropsychological evaluation current within 1 (one) year and be willing to provide The Crumley House with a copy.
-The individual must be in stable medical condition as evidenced in a current medical evaluation completed within 30 (thirty) days of the established admissions date.
-The individual must not exhibit any behavior which is dangerous or disruptive to the health or welfare of self or others.
-The individual must possess basic self-help skills such as toileting, eating, mobility, etc. or have supports which eliminates these barriers.
-Individual must have transportation or be willing to assist in making arrangements to obtain it.
-Individual must possess basic communication skills or have the ability to use adaptive communicative equipment.
-Admission can only be granted if space is available.
-Admissions will be granted to qualified applicants without regard to race, religion, sex, national origin, or political affiliation.
*Admissions to the day program are granted with an initial 30-day probationary/evaluation period to ensure appropriateness for client.*
